Insurance terms

Deductible - an amount that a patient must pay for their healthcare before their insurance company will pay anything.  This is usually reset every year.

Co-pay - the patient pays a small amount for office visits.  The amount depends on your insurance company.

Co-insurance - a percentage of your bill that you must pay after your deductible is met.  Your insurance company pays the other part.

Network - a group of providers and hospitals that your insurance has contracted with.  If your insurance company has a contract with a provider, they are in-network.  If they don't, they are out-of-network.  The insurance generally pays more for an in-network provider than an out-of-network one.

Assignment of Benefits - the provider bills the insurance company first and then bills the patient for the part of the charges that they must pay

Preauthorization - sometimes an insurance company requires that you get authorization before you have a procedure or hospital admission.  If you don't get preauthorization, the insurance company may not pay anything for those services.  Requirements for preauthorization depend on your insurance company and plan.
