Clinic Closure Notice

RE: Closure of Blue River Family Medicine Clinic

As a valued patient of Blue River Family Medicine, we regret to share with you the decision that has been made to close the clinic’s operations. This decision has not been taken lightly, as we are acutely aware of the impact it has on our staff, patients, and community.

The clinic’s last date of patient visits will be July 31, 2024. Until this date, we remain open and available to assist you with your healthcare needs, medication refills, and transferring your medical records to a provider of your choice.

BRFM will continue to maintain your medical records until July 31, 2024, and you can continue to access your health information in your patient portal until that time. Beginning August 1, the patient portal will not be available, but your medical record will be maintained by and available from Stormont Vail Health. If you wish to transfer your care outside of Stormont Vail Health after August 1, 2024, please contact their office at 785-537-2651.

Understandably, this is disappointing news; however, your provider is keeping a presence in the area. Dr. Graves, Dr. Shamburg, Samantha Davis, and Myra Wilson will be located at the Stormont Vail Health Manhattan Campus: 1325 Research Park Drive Manhattan, KS 66502.

We want to sincerely thank you for trusting us with your healthcare needs and regret having reached this difficult decision. We are here to help ease your transition to a new healthcare practice and will do everything we can to make the transition a smooth one.
